PHOTO, VIDEO, TESTIMONIAL, OTHER PUBLICATIONS

By applying for enrollment and beginning a PersoNell Wellness program, you hereby agree to submit progress photos (example: "Before" and "After" photos) to PersoNell Wellness according to the program you begin and as requested during the program you begin. PersoNell Wellness reserves the right to terminate all services provided to you at any time during the program if you are non-compliant.

You hereby agree and grant permission to Chaunell Walker, The ProfessioNellz, LLC, dba PersoNell Wellness (here on known as PersoNell Wellness) to use photos, videos, testimonials and any other publications, hereinafter referred to as “Materials, ” submitted to PersoNell Wellness for any PersoNell Wellness use (examples: website, social media, publications, literature, advertising, and marketing).

You hereby release PersoNell Wellness, its representatives, employees, managers, members, officers, parent companies, subsidiaries, and directors, from all claims and demands arising out of or in connection with any use of said “Materials,” including without limitation, all claims for invasion of privacy, infringement of your right of publicity, defamation, and any other personal and/or property rights.

You acknowledge and agree that no sums whatsoever will be due to you as a result of the use and/or exploration of the “Materials” or any rights therein. 

You acknowledge and agree that using any PersoNell Wellness products and/or services, signing this form, or replying by email, “I consent” or any indication of consent is giving your consent to the above.
